Preparation for the visit

- Have a referral for a Level III hematologist consultation.
- Write down your symptoms (when did they start, how long does they last, how and when do they occur)
- Write down any questions you want to ask your doctor during the visit.

During the visit

Provide your doctor with a list of all changes in your routine (for example: diet, exercise or activity, sleep habits).

Provide a list of all medications you are currently taking (vitamins, herbs, mineral supplements).
Write down your doctor's answers and instructions. If you have difficulty writing or think you are too concerned, consider bringing a family member or friend. If your doctor prescribes medications, make sure you understand their benefits and risks, as well as how to use and store them.

Follow - up after the visit

Contact us for more information. If you still have questions, feel free to call your coordinator or doctor and discuss them.
Report symptoms that occur after new treatments or medication changes.
Call at the agreed time for tests results.
