Preparation for the visit

  • Have a referral for a Level III hematologist consultation.
  • Write down your symptoms (when did they start, how long does they last, how and when do they occur)
  • Write down any questions you want to ask your doctor during the visit.

During the visit

  • Provide your doctor with a list of all changes in your routine (for example: diet, exercise or activity, sleep habits).

  • Provide a list of all medications you are currently taking (vitamins, herbs, mineral supplements).

  • Write down your doctor's answers and instructions. If you have difficulty writing or think you are too concerned, consider bringing a family member or friend. If your doctor prescribes medications, make sure you understand their benefits and risks, as well as how to use and store them.

Follow - up after the visit

  • Contact us for more information. If you still have questions, feel free to call your coordinator or doctor and discuss them.

  • Report symptoms that occur after new treatments or medication changes.

  • Call at the agreed time for tests results.
