Rheumatic diseases affect about 120 millions of Europe population. It takes the first or the second place, depending on the country, in the structure of disability of European countries. Consequently the demand of rheumatologist services is increasing every year. About 1000 patients are treated in rheumatology department annually and about 15 000 patients are consulted in rheumatologist consulting room. It is possible to perform a wide spectrum of immunological tests for the patients and implementation of new immunological tests enables to differentiate severe rheumatic diseases with similar clinical picture. The doctors of rheumatology center have a good command of joints sonography technique and some of them also can perform capilliaroscopy. Needle joint and soft tissue biopsy procedure and histological examination of the sample can also be performed. Patients with severe systemic connective tissue diseases can be treated with pulse therapy as well as immunosuppressants. New treatment method with biological therapy drugs was established in rheumatology practice since 2000. According to strict algorithms it is possible to prescribe the treatment to patients with severe form of rheumatoid arthritis, ankylosing spondylitis and psoriatic arthritis. It is really effective treatment method for the majority of patients. Although the number of patients treated with biologicals increases every year, it is still not possible to prescribe this effective treatment to every patient who needs it due to the lack of state funds and strict indications.

The progression of disease and joints anatomical structure damage can be stopped in many cases when diagnosis is determined early and appropriate treatment is prescribed in time. Therefore particularly relevant nowadays is the question of early undifferentiated arthritis, early diagnosis and proper treatment assignment at "window of opportunity" periods, in order to prevent further progression of inflammatory rheumatic disease and irreversible structural joint damage.
