Cardiac Intensive Care Department

Head – Giedrius Navickas
Chief Nurse Jadvyga Lozovska
Ist and IInd Departments of Cardiology every year provide treatment to patients suffering from angina pectoris, myocardial infarction, pulmonary embolism, disorders of cardiac rhythm and conduction, acute or exacerbated heart failure, pericardial and myocardial diseases.
The Cardiac Intensive Care Unit (CCU) provides care to patients suffering from acute coronary syndromes (myocardial infarction, angina), acute cardiac arrhythmia, acute heart failure, pulmonary artery thrombosis. To CCU there are admissions from 22 lithuanian hospitals with catchment area of 1,3 mln lithuanian population. The unit coordinates 3 ambulance teams (doctor and nurse) who are responsible for transfer of critically ill patients from different hospitals of the region. One of the teams is based in Vilnius, others - in Utena and Alytus hospitals. Patients normally are transferred back after intensive treatment for further care.
Pulmonary artery thrombosis is widely treated with thrombolysis , in case of contraindications for thrombolysis, there are invasive procedures available. Patients with acute heart failure are treated with intra aortic balloon pump, haemofiltration, haemodialysis. Therapeutic hypothermia after prolonged resuscitation is available since 2011 as well as ECMO procedures for critically ill patients with heart failure. Invasive and non-invasive hemodynamic monitoring, airway management, 24/7 echocardiography service is provided.
The unit contains 24 beds fully equipped with the facility for monitoring and airway management and able to treat around 4000 patients per year. The Department of Intensive Cardiology treats patients suffering from acute coronary syndromes, acute disorders of cardiac rhythm and conduction, acute heart failure, pulmonary embolism.

In the Sub-department of Intracardiac Treatment of Cardiac Arrhythmias every year 550 pacemakers, 15 cardioverter defibrillators are implanted. For the first time in Lithuania resinchronization treatment of patiens with extreme heart failure is started (by 2006 20 procedure have been carried out). Over a year, the Sub-department on average performs 250 transcatheter ablation procedures, and 80 invasive procedures for the treatment of atrial fibrillation combined with open heart surgery (in cooperation with the Clinic of Cardiac Surger).

Invasive cardiology unit and in-patient invasive cardiology unit

Head – MD, Ph.D. Giedrius Davidavičius
Chief Nurse Jurgina Klevėnienė

In the Sub-department of Interventional Cardiology all types of intracardiovascular diagnostic and treatment procedures are performed to neonates, children and adults. The largest experience in the country is accumulated in the area of transcatheter interventional procedures on coronary arteries: annually on average 5000 coronarographies and 1850 percutaneous angioplasties are performed, out of those, 1360 are finished with stent placement (about 70). The Sub-department performs the closing of atrial septal defects and implantation of anti-embolic filters in the inferior vena cava. Here, the vastest experience in the Baltic States has been agglomerated in the treatment of hypertrophic obstructive cardiomyopathy, by applying alcohol ventricle septal ablation (28 procedures - started in 1997). Here, procedures of stent placement in the descending aorta and balloon valvuloplasty have been carried out for the first time in the country. Moreover, here intracoronary and intracardiac ultrasound examinations have been started in Lithuania.

Head Ph.D., assoc. prof. Jonas Misiūra
Chief Nurse Roma Voleišienė

The Department of Non-invasive and Ultrasound Cardiovascular Diagnostics performs all types of ultrasound examinations, multi-dimensional computer tomography and magnetic resonance imaging on blood vestels, arteries and veins.

Unit of Adult Cardiovascular Ultrasound Diagnostics

Head dr., assoc. prof. Sigita Aidietienė

Established in 1982, the Echocardiography Laboratory (Echo Lab) served as the founding nucleus of The Department of Cardiovascular Non-invasive and Ultrasound Diagnostics. Transformed into the Unit of Adult Cardiovascular Ultrasound Diagnostics it still remains one of the largest clinical, educational and research echocardiographic laboratory facilities of its kind in Lithuania. Outpatient and inpatient echocardiography studies are routinely performed in the lab using state-of-the-art equipment.

The Echokg unit provides a comprehensive assessment of heart structure and function, as well as normal and abnormal blood flow within the heart. An echocardiogram is a noninvasive test that uses sound waves (ultrasound) to produce detailed images of the heart's size, structure and motion. An echocardiogram may also be used to measure the heart's blood volume and the speed and direction of blood flow through the heart.

The experienced team of cardiologists in the laboratory provides patients with the highest quality of care. The team performs more than 28 000 comprehensive patient examinations each year using transthoracic and transesophageal echocardiography. More than 100 tests are performed each day. Transthoracic, transesophageal, as well as intraoperative echocardiograms, are the tests most often performed in the laboratory.

Unit of Electrocardiography and Exercise Testing

Head Audrutė Dailydkienė

High volume of ECG recordings is produced in the Unit. An ECG may show evidence of a heart attack, lack of blood supply to your heart or heart rhythm disturbances. About half of ECGs are analysed and assessed by qualified specialists, not entirely by ECG machine. ECG stress tests are performing on a treadmill or stationary bicycle. Physicians at the Unit of Electrocardiography and Stress Testing use ECG and ECG stress testing to effectively diagnose potential cardiac conditions (rhytm disturbancies, coronary heart disease, heart insufficiency et cet.)

Unit of Stress Echocardiography and cardiopulmonary stress testing

Head dr. Jelena Čelutkienė

There are two rooms for stress echocardiography in the Unit equipped with semi-supine bicycle for exercise test and two high-end ultrasonic machines. Dobutamine and adenosine are being used for pharmacological stress, if needed images are acquired with echocontrast material. Stress tests are performed for detection of coronary insufficiency, myocardial viability, evaluation of heart failure severity, individual prognosis, differential diagnosis of dyspnea. The Unit is busy performing stress tests for in-patients and out-patients in cardiology and cardiac surgery departments.

Unit of Ultrasound and Non-invasive Cardiovascular Diagnostics of congenital heart diseases

Head dr. Daina Liekienė

The full carefull and versatile noninvasive examination of heart and great vessels for patients with congenital heart deseases (from newborns to adults) is performed in the Unit. From the beginning the Unit served as diagnostic facility before and after surgical correction of congenital heart diseases.Outpatient and inpatient Cardiac ultrasound (all modalities), ECG, Holter monitoring, ECG  stress testing studies are routinely performed in the lab using state-of-the-art equipment. The Unit works in close cooperation with heart surgeons and the Unit of pulmonary hypertension.

Unit of vascular non-invasive investigations

Head dr. L.Ryliškytė

The Unit carries our non-invasive vascular assessment, which includes ultrasound tests of peripheral arteries and veins, evaluations of endothelial function and arterial stiffness, and measurement of central blood pressure. Close collaboration with Unit of Preventive Cardiology is pursued.

Division of Cardiology Outpatient Consultations

Cardiologist, Ph.D, senior physician of the Division Jūratė Barysienė

The patients suffering from cardiovascular diseases are consulted and examined at this Division. The total of 65 000 consultations are being performed annually. The full scope of non-invasive examinations is being used for the patients, as well as the procedures performed at the Centre of Cardiology and Angiology. The patients are consulted in accordance with main cardiovascular disease, including acquired valve malformations, ischemic heart disease, heart rhythm disturbances, cardiomyopathies, arterial hypertension; the patients after heart operations or invasive procedures are observed there, too.

Preventitive Cardiology department

Chief ordinator Dr. Jolita Badarienė

Subdivision headings: Preventive Cardiology Subdivision, Primary Cardiology Subdivision.
Preventive Cardiology Subdivision involves primary prevention consultation room and secondary prevention subdivision. Primary cardiology prevention consultation room screens and follows up patients according to primary screening and prevention programme LitHir for high CVD risk group : men 40-55 years old, women 50-65 years old with metabolic syndrome or diabetes. Secondary Prevention Subdivision manages patients with coronary heart disease, stable angina pectoris, unstable angina pectoris, history of myocardial infarction, after coronary artery stenting or bypass surgery, as well as patients with persistent hypertension, metabolic syndrome, with severe congenital dyslipidemias. Patients are being consulted and followed up for three years after myocardial infarction, two years after percutaneus coronary artery stenting. Consultations are scheduled in advance with their treating doctor.

On arrival patients must have a referral.

Subdepartment for congenitital heart diseases in adult

Heard of subdepartment – PhD Lina Gumbienė

Adult patients with congenital heart disease (before and after correction), those with Marfan syndrome and other hereditary heart diseases as well as pulmonary hypertension are consulted and managed. Patients undergo a thorough ambulatoric examination and, if necessary, are hospitalized for an invasive examination or treatment (surgical or transcatheteric) and receive recommendation for follow-up. Women with congenital heart disease are consulted about pregnancy risks, and are monitored during pregnancy.Registration in advance is required. Patients must have a dispatch and test results.

Department of Cardiologic Rehabilitation

Head – Ph.D., assoc. prof. Alfredas Rudys
