The Centre of Laboratory Medicine located in the Building E of Vilnius University Hospital Santaros Klinikos.
It is in the middle of the main hospital locations: outpatients clinics (2nd floor), hospital itself (2nd floor) and the new hospital building.
Mission – to provide top quality laboratory diagnostics and consultation service.
Vision – to be progressive laboratory screening serving needs of patients, physician, scientific institutions and health care providers.
Centre of laboratory medicine is one of the biggest structural divisions in the hospital, also it is the biggest centre of such type in Lithuania. Centre is supervised by Prof., Ph.D., Habil. Dr. Zita Aušrelė Kučinskienė, Full Member of the Lithuanian Academy of Sciences.

Reception Of Laboratory Tests

Reception division receives orders for laboratory tests performed in all laboratories of CLM from the customers, then manages information, and forwards orders to the appropriate laboratory and test results to the customer site. Priority is given for the emergency tests.

Implementation of the unified sample-report logistic system serves the best for both laboratories and customers.

Flow cytometry subdivision

In this subdivision following procedures are carried out with flow cytometers: stem cell examination, acute and chronic leukemia/lymphoma diagnostic phenotyping, minimal residual disease assessment, paroxysmal nocturnal hemoglobinuria diagnostics. Donor blood component quality control tests are performed since 2011. Tests are performed with multiparametric flow cytometry methods of cell sorting for later in vitro examination capability.

Analytical hematology subdivision

Modern hematology analyzers are used to carry out quantitative and qualitative blood tests . Automatic hematology analyzers enable detection of more than 25 blood analytes. Selection for cytomorphologic blood smear test is performed with these analyzers.

Hematology (blood and blood marrow) cytology subdivision

Analyzes pathologic blood, blood marrow, trepanobioptate and lymph node aspirate samples, cytochemical reactions are performed to determine mieloperoxidase, non-specific esterase, iron reserves.

General cytology subdivision

Body fluid, gynecologic and various localization cytology tests are performed here. Body fluid cellular composition is determined in an automatic fashion, using specialized software. Liquid medium method LiquiPrep (USA) is used for gynecologic and non-gynecologic specimen preparation, which enables a broad spectrum of tests from the same sample.

Urinalysis subdivision

This subdivision has a closed urine collection system installed. Automatic urine tests are performed using diagnostic urine dipsticks. Microscopic urine analysis is performed using automatic microscopy analysis software. When significant changes are detected, urine sediment microscopic analysis is performed with light field microscope, using supravital staining. Chemical analysis of urinary tract stones is performed in this subdivision.

Bladder washings' micropreparations are prepared using cytocentrifuge method, which enables to achieve highest quality of smear preparation.

 

2. Laboratory Of Clinical Chemistry

 Laboratory offer widest spectrum of clinical chemistry tests and consultations with the highest quality for 24 hours a day all year round.

Laboratory of clinical chemistry the first laboratory in Lithuania implemented consolidation, integration and process improvement project by LEAN, covering automated pre-analytic sample preparation, improved information flow and integrated analytical systems.  

Permanently improving analytical quality, laboratory became the best known laboratory of clinical chemistry in the country. Almost two decades it is also known as a laboratory testing site for clinical trials.

In the Division of Routine Clinical Chemistry, Immunochemistry and Coagulation investigations of metabolites, electrolytes, enzymes, lipids, carbohydrates, hormones, cardiac, anemia, tumor markers, specific proteins, coagulation factors, platelet function, therapeutic drug monitoring and drugs of abuse are performed. In the Division of Immunochemical Markers of Infectious diseases investigations of viruses’ antibodies and antigens of fungus are carried out. Phone: +370 688 62493.

Division of Electrophoresis and Chromatography operate electrophoresis, immunofixation, isoelectrofucusing, HPLC and MS/MS technics for investigation of proteins, lipoproteins, isoenzymes, drugs, metabolites. Phone: +370 688 62864.

3. LABORATORY OF MICROBIOLOGY

The Microbiology Lab performs clinical microbiology tests that help to identify the causes of infectious diseases. In addition, the laboratory performs sensitivity testing of bacteria and yeasts to antimicrobials in order to select the best antimicrobial drugs for treatment.

Testing Material: Clinical samples: smears from the respiratory tract, the skin, wounds, pus, faeces, sputum, urine, blood, cerebrospinal fluid and other biological body fluids, operating materials. Laboratory performs microbiological control of hospital environment

Identifiable Microorganisms: Infectious agents that can be grown in culture such as β- haemolytic Streptococcus, Streptococcus pneumoniae, Staphylococcus aureus, Haemophillus influenzae, Listeria monocytogenes, Pseudomonas aeruginosa, Escherichia coli, and other important bacteria. The lab can isolate pathogenic intestinal bacteria such as Salmonella, Shigella, Yersinia, and Campylobacter; detect rotavirus and adenovirus antigens, Clostridium difficile toxins. Also, anaerobic infectious agents such as Clostridium, Bacteroides, Fusobacterium,  Actinomyces, Peptostreptococcus and other bacterium genera can be grown in cultures for identification.

The lab can isolate and identify more than 100 species of rare pathogenic or opportunistic pathogens in immunocompromised patients, test for STD agents like Neisseria gonorrhoeae, Mycoplasma hominis, Ureaplazma urealyticum.

Clinically important fungi that cause mycoses can be isolated and identified to species level (e.g. Candida species, molds, dermatophytes, species causing microsporidiosis, some rare fungi species).

Tests for microbial sensitivity to antibiotic drugs: Performed for bacteria and yeasts in regards to patient’s state and after discussion with patient’s doctor. The widespread antibiotic-resistant bacterial strains are detected and registered: MRSA (methicillin-resistant S. aureus), VRE (vancomycin-resistant Enterococci), PNSP (penicillin-non susceptible S.pneumoniae), ESBL (extended-spectrum beta-lactamase) producing enterobacteria, also species of Pseudomonas and Acinetobacter that show multiple resistances to antibiotics.

Laboratory participates in European surveillance network EARSS, external quality assurance programs.

 

4. LABORATORY OF CLINICAL IMMUNOLOGY

The following tests and test groups are being performed at the laboratory:

Tissue Typing, provided on round the clock basis for both organ and allogenic haematopoetic stem cell transplantation programs. Total workload consists of typing for organ recipients, live and cadaver donors, providing the National Transplant Waiting List, including pre- and post-transplant sensitisation testing, accumulation of serum samples bank for cross-matching, as well as primary and control typing of patients and their families for allogeneic haematopoetic stem cell transplantation, and HLA typing and data management for Lithuanian National Bone Marrow Donor Registry.

Flow cytometry and cell function analysis is provided for pre-transplant recipient-donor cross-matching and post-transplant immune status monitoring, as well as for HLA-B27, basophile degranulation testing, immunological studies on bronchoalveolar lavage fluid, etc.

Autoimmunity testing and allergen-specific IgE class antibodies detecting is being performed using immunofluorescence technique, enzyme immunoassay and immunoblotting methods. These methods are in use for some infections testing (e.g., Borrelia burgdorferi serology, QuantiFERON®-TB Gold In-Tube diagnostic test for Mycobacterium tuberculosis infection) as well.

 

5. BLOOD TRANSFUSION LABORATORY

Laboratory staff consists of qualified laboratory medicine physicians, medical biologists and laboratory technologists.

There is constant proficiency improvement, laboratory has implemented internal test quality control procedures and also participates in international proficiency testing shedules Labquality (Finland).

All emergency tests are carried out day and night not only for the hospital patients, but also for patients in the hospital in Vilnius and in the whole country.

There the following groups of tests performed:
1.Blood and bone marrow donation. There are a few tests for donors:
   Blood grouping (ABO, Rh D, Kell antigens; RhD VI category, if necessary);
   Antibody screening.
2.Tests for recipients:
   Blood grouping (ABO, RhD);
   Antibody screening.
3.Crossmatch reactions:
   To avoid complications during or after transfusion, recipient should get compatible blood. It is performed by indirect antiglobulin test.
4.Antibody identification:
   DiaMed micro typing system and standart erythrocytes are used to identify antibodies.
5.Determination of other erythrocyte antigen systems:
   Rh, Kell, Duffy, Kidd, Lewis, P, MNS, Lutheran
6.Direct antiglobulin test:
   A positive DAT generally indicates that the red cells are coated in vivo with immunoglobulin and/or complement. There are several ID card for DAT and that allows differentiation between immunoglobulins or complement coating red cells.
7.Antibody screening and identificatin during pregnancy:
   Antibodies in mothers blood can cause several complications. So, it is necessary to identify them and choose a right treatment.
8.Blood grooping and direct antiglobulin test for newborns.
9. Other tests: cold antibodies, anti-heparin antibodies determination.

 

6. THE LABORATORY OF MOLECULAR DIAGNOSTIC

In the Laboratory of molecular diagnostics molecular tests are performed on various clinical samples (blood, urine, bronchoalveolar lavage, amniotic and cerebrospinal fluid, stool, brushes and swabs from urogenital and respiratory tract) in order to detect different infectious agents:

  • Viruses (CMV, EBV, hepatitis B, hepatitis C and HCV genotyping, high-risk HPV types, polyoma viruses (JC and BK), adenoviruses, herpes viruses (Herpes 1/2, HHV-6 and HHV-8), influenza (A, B and pandemic H1N1).
  • Bacterial meningitis pathogens from CSF – N. meningitidis, H. influenzae and S.pneumoniae.
  • Toxoplasma gondii in the amniotic fluid and blood.
  • STI pathogens (Chlamydia trachomatis, Neisseria gonorrhoeae, Mycoplasma genitalium, Ureaplasma urealyticum, Trichomonas vaginalis).
  • Sepsis causing pathogens.
  • C. difficile from stool.
  • Combined testing of nucleic acids (NAT) of blood-borne viruses (HCV, HBV and HIV 1/2) for blood donors.

Low/middle resolution HLA (I and II classes) typing is performed for organ or bone marrow transplantations.
