Kestutis Rucinskas
1st Unit of Cardiac Surgery. Head of the unit Dr. Kęstutis Ručinskas PhD

400 cardiac surgeries are performed and near 800 patients are treated annually. Valvular pathology, coronary heart disease, aortic pathology, cardiac tumours are major fields of treatment. Surgeons in the department performs whole spectrum of surgical valve repair or replacement, coronary arteries bypass grafting, surgical treatment of aortic dilatation and dissection and combined procedures. Number of less invasive heart valve repair/replacement procedures is growing. Mitral valve repair with neochords without cardiopulmonary bypass is the most modern surgical method developed in the department. The department also specializes in treatment of patients with terminal heart failure and provides Heart transplantation and cardiac assist device implantation services. Team of specialized cardiologists: G.Maneikienė, P.Semėnienė, G.Zuozienė, R.Čypienė and clinical imunologists: R.Malickaitė ir L.Jurgauskienė provides diagnostic and medical treatment needs.

2nd Unit of Cardiac Surgery. Head of the unit Dr. Virgilijus Lebetkevičius

In this unit about 350 children are treated, and over 200 operations are performed annually. This is the only department of its kind in Lithuania. The main type of work is surgical treatment of patients with congenital heart defects. All kinds of emergency and elective congenital heart disease surgeries are performed to patients range from neonatal to adulthood. Surgeries include correction of total anomalous pulmonary venous connection, hypoplastic left heart syndrome, transposition of the great arteries, atrioventricular communication, tetralogy of Fallot, various forms of coarctation of the aorta, interrupted aortic arch, atrial or ventricular septal defects, unique Ross operation. In case of no possibility to create two ventricular heart physiology one ventricular circulation type surgeries are performed. Significant amount of work consists of redo surgeries (for patients with implanted valves or conduits which become too small or one ventricular type repair). Atrial septal defects, partial anomalous pulmonary venous drainage are operated through minimal surgical approach without incision in the frontal chest plane. Rare children's heart surgeries are performed also: removal of the heart tumours, pacemaker implantation for small babies.

Children with advanced heart failure receive intensive treatment with medications, heart transplantation and implantation of heart support devices are performed also.

Some of the congenital heart pathology is treated with percutaneous invasive methods without conventional heart surgeries. A proper anatomy atrial septal defect, patent arterial duct, aortic coarctation are the most common defects for such procedures which are performed by interventional radiologists.

Children with heart rhythm and conduction disorders are treated by cardiac electrophysiologists.

Patients are treated and consulted by cardiologists R. Sudikienė, L. Liekienė, and K. Lankutis, as well as clinical immunologists R. Malickaitė and L. Jurgauskienė.

3ed Unit of Cardiac Surgery. Head of the Unit Dr. Gintaras Kalinauskas

The Third Department of Cardiac Surgery treats about 1300 patients and performs over 700 operations annually. The brunt of its work involves the surgical treatment of ischemic cardiac disease and its complications. The department has accumulated huge experience in this area. In addition to typical coronary artery bypass grafting operations, it performs beating heart bypass surgery, and using small incisions. It uses improved Jatene-Dor method to make resections of post-infarction aneurisms of the left ventricle and of the remodeled left ventricle. We performed the world’s first minimally invasive plastic operation on the left ventricle aneurysm and of the remodeled left ventricle on the beating heart. For the introduction of this method, Department Head G.Kalinauskas was awarded the Grand Cross of the Order for Merits to Lithuania. The department also performs plastic operations on postinfarction ventricular septal ruptures, prosthetic valve implant and valve plastic procedures, and combined valve and coronary artery bypass grafting operations. It handles patients with complex thoracic aorta pathologies, patients with acute thromboembolisms, Ross operations, and cardiac tumour operations. Combined electrophysiological and surgical procedures are performed in conjunction with interventional cardiologists. Due to advanced tumours, extended lung resections and cardiac plastic operations with cardiopulmonary bypass and oncological surgeons have been introduced. When advanced renal tumours exist, extended renal tumour operations and cardiac plastic operations using a cardiopulmonary bypass are performed together with urolological surgeons. The only place in the Baltic States where pulmonary arterial endarterectomy operations are performed is the Third Department of Cardiac Surgery. It additionally operates on and treats adult patients with congenital heart defects. Because no specialized congenital heart defect cardiology department exists in Lithuania, it also treats various congenital heart defect complications: heart arrhythmias, heart failure, and pulmonary hypertension. In addition, adolescent, adult, and pregnant patients are hospitalized here. The annual number of operations is expected to increase to 1000 and the introduction of endoscopic harvesting methods for arteries and veins is planned. Training with residents, students, and foreign heart surgeons fellows is regularly conducted in the department. Patients are treated and seen by heart surgeons and cardiologists with the highest qualifications: Ass. Prof. L.Ivaškevičienė, E.Voluckienė, E.Gateliene, D. Liekienė, L. Gumbienė, and L.Lankutienė. Our department’s physicians: G.Kalinauskas, G.Norkūnas, L.Ivaškevičienė, A.Valaika, and J.Verižnikovas were awarded a National Science Prize for the introduction and improvement of the surgical treatment of ischemic heart disease.

4th Unit of Cardiac Surgery. Head of the Unit Dr. Dobilas Barysas

The patients suffering from wound infections after heart surgery are being treated at the Department. About 50 patients needs a surgical wound revision and treatment annually.
