Who are we

The Cardiology and Angiology Center is an advanced, open for innovations, highly specialized center with a great clinical experience. Modern cardiovascular disease tests and treatment are applied here. The Center works closely with the Center of Heart and Chest surgery in order to fulfill its all functions. The staff of the Cardiology and Angiology Center is one of the best in Lithuania, having extremely high requirements for their qualification and quality of medical services.

What makes us unique

These modern interventional / minimally invasive procedures are performed in the Cardiology and Angiology Center:

  • For the first time in the world minimally invasive artificial chords were used to treat mitral regurgitation without the cardiopulmonary bypass in 2013.
  • These operations were performed for the first time in Lithuania: transcatheter aortic valve implantation (since 2009) through the femoral artery and through the apex of the heart, transcatheter thrombectomy during a thromboembolic event of the pulmonary artery,  transcatheter pulmonary artery valve implantation through the femoral vein, transcatheter mitral valve balloon valvuloplasty, transcatheter closure of the mitral valve paravalvular fistulas (since 2013) through the apex of the heart, stenting procedure of the descending aorta.
  • Innovations in the field of intensive cardiology are applied since 2011 – therapeutic hypothermia and extracorporeal membrane oxygenation (ECMO) in collaboration with the Center of Heart and Chest surgery.  
  • The Cardiology and Angiology Center was the first in Lithuania to apply intracoronary and intracardiac ultrasound tests. A fully functioning modern pulmonary hypertension coordination Center was founded in 2010 and it was the first one in the country.  
  • The latest electrophysiological procedures are performed in the Cardiology and Angiology Center: radiofrequency catheter ablation was performed for the first time in Lithuania; transcatheter pulmonary vein isolation with radiofrequency ablation (using three-dimensional electromagnetic cartography system) and cryoballoon catheters, surgical treatment of atrial fibrillation - "The labyrinth" procedure with cardiopulmonary bypass or minimally invasive surgical treatment of atrial fibrillation without cardiopulmonary bypass.  
  • Modern complex treatment of heart failure is applied in the Center – from drugs to heart resynchronisation therapy and, in collaboration with the Center of Heart and Chest surgery, patients are prepared for a heart transplantation operation and are treated afterwards. The Cardiology and Angiology Center is the leader in Lithuania according to the number of implantable devices (pacemakers and cardioverters – defibrillators) every year.  The Cardiology and Angiology Center is the only center in Lithuania, in which cardiac pacemakers are implanted to newborn babies and young children.      

What we offer to the patients

The staff of the Cardiology and Angiology Center consists of competent professionals who, in addition to the daily clinical work, take part in international medical and biomedical surveys, scientific experimental studies, international registries and scientific-practical conferences in Lithuania and abroad.  

The most innovative cardiovascular research and treatment methods are applied and a highly qualified team of professionals is working in the Center.

The Cardiology and Angiology Center consists of:

  • Intensive cardiac care unit (24 beds, about 4.000 patients are treated per year in this department);
  • 1st and 2nd Cardiology departments (56 beds to treat patients with a variety of chronic and acute heart diseases); Cardiac arrhythmia and radiosurgery department (here interventional coronary artery and electrophysiological procedures are performed, interventional cardiology inpatient section (32 beds) belongs to this department);
  • Cardiovascular non-invasive and ultrasound diagnostic department (electrocardiography, perthoracic and transesophageal echocardiography tests, electrocardiographic exercise tests (veloergometry, treadmill), ultrasound stress tests (pharmacological and exercise tests), contrast echocardiography, cardiopulmonary exercise test, visual stress tests, complex evaluation of the vascular endothelial function, non-invasive peripheral vascular examination);
  • Outpatient cardiology department (patients with all congenital and acquired cardiac and vascular diseases are consulted and examinated; about 65.000 children and adults are consulted  over the year);
  • Cardiology Rehabilitation department (32 beds).

Such procedures like esophageal electrophysiological studies, survey of implanted pacemakers and cardioverters – defibrillators, electrocardiograms and monitoring of the blood pressure are performed all day (outpatient treatment). The Cardiology and Angiology Center offers excellent patient care and nursing, patient-sensitive environment.

What we offer to the medical society 

In the Center of Cardiology and Angiology students of the Faculty of Medicine of Vilnius University study and doctors improve their qualification in Lithuanian, Russian and English languages. The center is a post graduate base, in which residency and doctoral  studies take place continually. Here international conferences with current topics involving highly qualified Lithuanian and foreign lecturers are organised. The Center offers the opportunity to gain not only theoretical but also practical skills of cardiovascular disease diagnostics and treatment.

History of the Centre

The Centre of Cardiology and Angiology (hereinafter referred to as the Centre) derives from the Departments of Cardiology and General Surgery of Vilnius University, Laboratory of Application-oriented Cardiovascular Testing and their clinical bases - clinical sub-divisions of Antakalnis and Red Cross Hospitals of Vilnius City.

The Department of Cardiology (Head - prof. J. Rugienius) was established in 1977 instead of the Department of Hospital Therapy (headed by prof. L. Laucevicius). In 1980, it was moved to Vilnius Republican Clinical Hospital (current Santaros Klinikos) and according to the agreement it used the following cardiologic sub-divisions of this Hospital for training and science purposes: the Department of Cardiologic Reanimation (Head - doct. A. Jukneviciene), two Departments of Cardiology (headed by assoc. prof. R. Vidugiriene and doct. D. Zinkeviciute), the Department of Functional Diagnostics (Head - doct. J. Lasiene) and the Consultative Out-patient Department. In the Department of Cardiology the following procedures were performed: subclavian vein microcatheterization without X-ray control, registration of haemodynamic and electrochemical parameters of the heart right-side cavities (platinum and hydrogen test) . The innovative work related to the His' bundle, synus node and atrial electrography, temporary diagnostic and therapeutic electrocardiostimulation was performed as well (prof. J. Rugienius, prof. V. Zvironaite, doct. S. Korablikovas and others). Workers of the Department patented the methodology for registering differentiated enhanced ECG (J. Rugienius, A. Sakalas, V Zvironaite). For the treatment of patients after myocardial infarction, intracoronary and intravenous trombolysis was started to apply alongside routine conservative treatment.

In the Department, 1 habilitate doctor's thesis (V. Zvironaite) and 6 dissertations of doctor of medicine sciences have been maintained (B. Abraitiene, L. Grigoniene, A. Kibarskis, B Petrauskiene, A. Rudys, R. Steponeniene).

The Department of General Surgery and Application-oriented Cardiovascular Testing Laboratory (headed by the academician prof. A. Marcinkevicius) are important to the development of cardiosurgery and cardiology of Vilnius and Lithuania as well as the former USSR and Eastern Europe. Alongside broad spectrum of cardiovascular surgeries, here the new generation of cardiologists was being raised, methodologies of interventional radiology and angiography for diagnostics and transcatheter treatment were implemented (hab. dr. A. Storpirstyte, assoc. prof. E. Kosinskas and others), pacemakers implanted (E. Kosinskas). The first intracardiac electrophysiologists (A. Aidietis, A. Laucevicius) reached their maturity here. The Vilnius Clinic of Cardiosurgery started first applying contemporary one- and two-dimensional, Doppler and echocontrast echocardiography, and computer processing of ehocardiogrammes (assoc. prof. V. Cobotas, assoc. prof. J. Misiura, A. Laucevicius).

In 1990, upon the merger of the divisions of University and the Hospital, the Clinic of Cardiology was formed, which was headed by prof. A. Laucevicius. Later on, in 2002, the Clinic of Cardiology was reorganized into the Clinic of Cardiovascular Diseases (Head - prof. A. Laucevicius), whilst the Centre of Cardiology and Angiology was established in Vilnius University Hospital Santaros Klinikos (Santariskiu Klinikos), the Manager of which was prof. A. Laucevicius, and the (ieneral Administrator - assoc. prof. B. Abraitiene. Since 1990, 1 habilitate doctor's dissertation (L. Ivaskeviciene) and 22 dissertations for a doctor of medicine degree have been maintained.

In 2002 on the base of Clinic of Cardiology was esteblished Clinics of Cardiac and Vascular Diseases (part department of Vilnius University Faculty of Medicine). Within Vilnius University Hospital Santaros Klinikos (Santariškiu Klinikos) Center of Cardiology and Angiology was founded. Prof. A.Laucevicius headed the Centre since 2002 to 2009, then prof. A.Aidietis became the head of the Centre. 

In 1997, under the supervision of prof. A. Laucevičius and prof. Ž. Petrulionienė a Preventitive Cardiology subdivision was founded. It was the first subdivision of this kind in the Baltic States, and one of the first in Eastern Europe. After the establishment of Cardiology and Angiology center, this subdivision in conjunction with other cardiac subdivisions became part of the center. Currently the head of the subdivision is med. Dr. Jolita Badarienė. Since its establishment of this subdivision patients with myocardial infarction, unstable angina and patients after coronary revascularization procedures as well as rare congenital dislipidemias have been managed. 

On the initiative of the European Society of Cardiology a retrospective study EUROASPIRE III (European Action on Secondary and Primary prevention by Intervention to Reduce Events) was carried out in this center during 2006-2007. The study EUROASPIRE III included patients with coronary heart disease, assessing their lifestyle, risk factors and use of cardioprotective drugs. The objective of the study was to clarify effectiveness of ongoing heart and cardiovascular disease treatment and prevention, rationality of the health policy. Lithuania was one of the 22 European Union countries where the study was carried out. 

Since 2005 patients with high cardiovascular disease risk have been consulted in the Primary prevention subdivision. The initiative has been funded by an ongoing primary screening and prevention program LitHir. The aim of the program is to reduce the incidence of cardiovascular disease, detect early stages of atherosclerosis in a very high-risk patient group with metabolic syndrome or diabetes. National Health Insurance Fund rated this program as one of the most effective ones having been carried out in Lithuania. In the department of Cardiac arrhythmias and invasive cardiology, the most advanced procedures for the treatment of various cardiac arrhythmias are performed. It is the only centre in Lithuania where cardiac pacemakers are implanted to newborns and small children. 

The history of radiofrequency catheter ablation of cardiac arrhythmias in Lithuania started in 1991, shortly after the first world publications on this topic have emerged. In our centre, first ablations in Lithuania have been performed: in 1991, procedures of atrioventricular nodal tachycardia and accessory pathway (Kent bundle) ablation have started, the first catheter ablation on atrial fibrillation (1999), the first cryoballoon catheter ablation of atrial fibrillation (2008). In collaboration with cardiac surgeons, modified radiofrequency atrial Maze procedures are performed. In 2009, minimally invasive surgical procedures for atrial fibrillation have been started, and the number of procedures are biggest in the Baltic states. 

The centre also has the biggest numbers in the country for the coronary artery interventions. Other types of interventional procedures are also performed: catheter closure of atrial septal defects, closure of the left atrial appendage, inferior caval vein filter implantations. In Lithuania, first procedures of stenting of descending aorta and balloon valvuloplasty have been started here, as intracoronary and intracardiac ultrasound diagnostic procedures. For the treatment of heart failure accompanied by conduction abnormalities, cardiac resynchronization therapy is used. The first biventricular pacemaker (CRT-P device) in Lithuania has been implanted on our centre in 2002, and the first biventricular defibrillator (CRT-D device) - in 2008.
